Experimental and analytical toxicology

The basic objective of research of the group is to increase knowledge and understanding of the human genetic variability and its toxicological, medical and forensic consequences.

Inflamatory arthropathies

The aim of the group is to promote research in the most prevalent chronic arthritis, rheumatoid arthritis and spondylarthritis. The main lines of research are: - Prognostic factors in recent onset arthritis.- Antibodies against citrulline-containing peptides in rheumatoid arthritis: diagnostic and prognostic value. - Angiogenesis in recent onset arthritis Prognostic value of angiogenesis; effect of biological therapies on synovial angiogenesis. - Synovial neolymphogenesis in rheumatoid arthritis and psoriatic arthritis. - Intermittent arthritis. Clinical and immunogenetic characterisation. - Efficiency and safety of biological therapies (TNF antagonists) in rheumatoid arthritis and spondyloarthropathies.

Systemic Autoimmune Diseases

Systemic autoimmune diseases group promotes basic and clinical research on systemic autoimmune diseases from a multidisciplinary point of view, with the participation of members of Autoimmune Diseases, Nephrology, Immunology, Haemotherapy, Hepatology and Dermatology Coordination Services. However, these activities are carried out in collaboration with various international work groups promoted by the European Union: European Working Party on Systemic Lupus Erythematosus (EWPSLE), European Forum on Antiphospholipid Antibodies (EFAPL), and the Systemic Lupus International Centres(SLICC). The CAPS Registry, coordinated by our group, has also been started. It is devoted to the study of the catastrophic antiphospholipid antibody syndrome. New lines of research concern the experimental model in mice, autoimmune manifestations induced by HCV, pathogenic mechanisms of systemic lupus erythematosus, Sjögren's syndrome and assessment of the cardiovascular risk factors of these diseases.

Infectious diseases and aids

Lines of research in the field of AIDS: - Immunotherapy of the disease, natural and acquired defence mechanisms, including the development of therapeutic vaccines. - Efficiency, mechanisms of resistance and tolerance of new antiretroviral drugs. - Immunopathogenicity, response to treatment and resistance mechanisms of some OI¿s including tuberculosis, PCP and toxoplasmosis. Lines of research in the field of general infections: - Prophylaxis and support to treatment of endocarditis caused by multiresistant germs, including experimental models. - Infections in solid organ transplant recipients. - Interactions between nosocomial infections and the use of antibiotics. - Pathogenicity and diagnostic markers of recurrent urinary tract infection. - Diagnosis of respiratory viruses in hospital-acquired and community-acquired pneumonia. Members of the Infectious Diseases and Microbiology Unit of Hospital Clínic de Barcelona, belong to these lines of research. Their research activity is primarily focused in AIDS and includes most of the research activity concerning infectious diseases and microbiology.

Epidemiology and international health research group

Our group has unified the former Research Group on Epidemiology and International Health and Research Group on Public Health and Nosocomial Infections. The new group has the aim to conduct research involving public health, epidemiology, prevention and control of transmittable diseases. One of the main pillars of our activity is the fight against the main causes of morbidity and mortality affecting children under 5 years and pregnant women in Sub-Saharan Africa. Most of the research lines are devoted to diseases defined as related to poorness, but there is also a line devoted to diseases imported to Spain and their epidemiological surveillance.

The strategic objective is the study of the inflammatory response in general. The main lines of research are: - Oxidative stress and its relation with diet. - Mechanisms involved in the development of systemic inflammatory processes. - Study of idiopathic pulmonary fibrosis. - Cellular, renal and intestinal hypoxia. - Mechanisms of the protective effect of ischemic preconditioning.

Atherosclerosis and coronary disease

Electrophysiology

The team are interested in three research objectives: - Firstly, to continue to develop research projects connected to prevalent pathologies, with a special interest in new therapies in the field of arrhythmia and cardiac stimulation. - Secondly, to develop the study line on channelopathies, expanding research in Brugadas Syndrome to other channelopathies such as long QT Syndrome. - Lastly, we want to initiate a basic research line with animal experimentation to create animal models in order to answer clinical challenges.

Nephro-urological disease and kidney transplantation

To get a better understanding of the physiopathology and treatment of kidney, urinary apparatus and masculine reproductive apparatus diseases and of medical complications stemming from the substitutive kidney treatment with dialysis of transplant. Our main aims are the study of immunosupression in pharmacokinetics and pharmacodynamic aspects. Furthermore, with the incorporation of a group specialised in transplant immunology, our objectives are also to achieve a better tolerance to allograft and xenograft, and to reduce the need for immunodepressants. Another
challenge is to go deeper into the knowledge related to the achieved degree of immunodepression in patients with respect to their needs: pharmacokinetics/ pharmacodynamics rate.

Hypertension, lipids and cardiovascular risk

The main objective is to increase knowledge and understanding of epidemiology, pathogenesis, diagnosis, treatment and prevention of dyslipidemia. Our current research lines are: - Characterisation of genetic dyslipidemia. - Detection of pre-clinical atherosclerosis. - Mechanisms and consequences of silent lesion of target organs in hypertension. - Interaction of environmental determinants in the cardiovascular disease such as salt consumption and the phenomenon of salt sensitivity. - Candidate genes in hypertension and their complications. - Study of cardiovascular functionality of food. - Study of intestinal cholesterol absorption, and assessment of mild cognitive impairment in dyslipidemia.

Respiratory biophysics and bioengineering

The general objectives of this group are the study of key biophysical mechanisms of the respiratory function and development of biomedical image and signal processing models and methods. This group is part of the recently created Catalan Bioengineering Reference Centre. The work is based in a multidisciplinary approach and the integration of basic and clinical research. In parallel to the scientific objectives, our group promotes the transference of technologic advances stemming from research to industry.

Management and prevention of infectious, interstitial and pulmonary tumor diseases

The objectives of research of this group are: - Study of aetiopathogenesis, diagnosis, treatment and prevention of community and hospital respiratory infections in humans and experimental bodies in immunodepression and in pulmonary tuberculosis. - Study of epidemiology and treatments of severe respiratory insufficiency, particularly the acute respiratory distress syndrome (ARDS) and its relation to respiratory infections and sepsis. - Study of invasive and non-invasive mechanic ventilation in humans and in animal model. Physiopathological aspects, clinical aspects, cost-effectiveness. - Study of the Chronic Obstructive Pulmonary Disease (COPD) in exacerbation. Involvement of pulmonary infections. - Study of pharmacokinetics and pharmacodynamics of antibiotics and other drugs in respiratory patients and in porcine animal model. - Study of the respiratory infection in Mozambique in collaboration with the Assessment, Support and Prevention Unit in IDIBAPS-Hospital Clínic. - Study of microorganism resistances to antituberculosis drugs and antibiotics. - Study of tuberculosis transmission.

Physiopathological mechanisms of respiratory diseases

The objectives of the group are: - Study of the molecular and cellular basis of the chronic respiratory diseases, with special emphasis in the disorders of pulmonary circulation and in extrapulmonary manifestations. - Study of the physiopathological and pathogenic mechanisms related to acute and chronic respiratory insufficiency and to respiratory sleep disorders, and hepatic and pulmonary disorders. - Development of models of artificial and bioartificial lung for intra- and extrahospital support for patients with respiratory insufficiency. - Study of determinant factors of clinical evolution and utilisation of health resources in chronic respiratory diseases.

Clinical and experimental respiratory immunoallergy

The main goal of the team is to conduct research on mechanisms regulating inflammatory response and remodelling in bronchial asthma, rhinosinusitis and interstitial pulmonary diseases. The lines of research are: - Molecular and cellular basis of the inflammatory response in asthma, rhinosinusitis and pulmonary interstitial diseases. - Role of metabolites of the arachidonic acid (prostaglandins and leukotrienes) in the aetiopathogenesis of asthma, chronic rhinosinusitis and pulmonary interstitial diseases. - Study of mechanisms regulating the response to treatment with glucocorticoids in respiratory inflammatory diseases. - Study of mechanisms involved in severe asthma. - New therapeutic modalities (endothelin, proteasome and COX-2 inhibitors, E2 prostaglandin 3 receptor agonists) in asthma, rhinosinusitis and interstitial pulmonary diseases, using animal models of asthma and pulmonary fibrosis.

Foetal and perinatal medicine

Our main strategic objective, and most of our resources, are devoted to the development of infrastructure and stable multidiscipline research lines for the prediction and prevention of neurological damage caused by foetal hypoxia and/or inflammation. These research lines have the collaboration of Foetal Medicine, Neonatology, Neuroimaging, Neuropaediatry and Molecular Biology groups. Other objectives of our group are the development and consolidation of lines existing in other fields such as perinatal infections, with an especial interest in HIV and neonatal infection, therapy and foetal surgery, screening strategies and prediction of foetal and mother pathologies in large populations, and prematureness.

Hepatic oncology

The aim of the group is to study the clinical and therapeutic aspects of hepatic cancer and other basic aspects. Our research activity is always concerned in the mechanisms regulating the progression of this disease. At a clinical level, work is carried out in the definition of optimal criteria for prescribing radical treatment (surgical resection, transplant from dead donor or living donor, percutaneous approach with alcohol injection or thermal ablation) to achieve the maximal efficiency in terms of both survival and associated morbidity.

The objective of research of the group is the determination of intracellular signalling and role of mitochondrion in the cellular susceptibility in processes of inflammation, hypoxia, stress, and the regulating role of cholesterol and sphingolipids, as well as to study the role of mitochondrial GSH in the regulation of NF-kB and HIF-1a through the modulation of respiratory chain free radicals generation. Cerebral ischemia: clinical and experimental studies

The aim of the group is to study cerebral ischemic pathology from both clinical and basic research points of view, with a special emphasis on inflammation and neuroprotection, as well as to identify histopathological, biochemical and molecular correlates of changes detected by neuroimaging. The lines of research: - Disorders of the immune system related to stroke and risk of infection. - Beneficial effect of antioxidative agent in ischemic stroke. - Inflammation and neurological impairment in the ischemic cerebral vascular accident (CVA). - Genetics of ischemic stroke and haemorrhagic stroke Prevention of infection in stroke patients. Neuropharmacology and experimental neuropathology

To have a better understanding of physiopathological mechanisms of psychiatric and neurological diseases, as well as neurotoxicity and ageing processes in the CNS. Our aim is to identify new cellular and functional targets and to propose new therapies. The main lines of research are: - Study on the glial transcription factors as possible therapeutic targets in the treatment of the neurodegenerative disease. - Role of glia in neuroprotection and in neuronal death. Study of how the induction of the glial activation can modify neuronal survival and how glial cells are activated according to the induced neuronal death pattern. - Study on cytotoxicity, oxidative stress, synaptic functions and proteomics to determinate altered targets in toxicity and ageing processes of CNS. Strategies of neuroprotection. - Control of neuronal proliferation. - Study on the neuronal circuitry responsible for the physiopathology and treatment of depression and schizophrenia; focusing in prefrontal cortex. - Study on neurotransmission and its anatomic, cellular, and molecular basis involved in neurodegenerative and psychiatric diseases, in order to identify and to characterize new sites of therapeutic action.

Degenerative and autoimmune diseases of the nervous system

The objective of the group is to develop research projects with results that can be applied in the short term to the improvement of diagnosis and treatment of degenerative and autoimmune neurological diseases. Thus, most of our research is linked to clinical research protocols of the Neurology Unit of Hospital Clínic. The main lines of research include: - Study on experimental models of the physiopathological mechanisms involved in the Parkinson's disease, as well as other movement disorders. - Study on autoimmune mechanisms of neurological diseases related to cancer. - Genetic and expression studies of Parkinson¿s disease, Alzheimer's disease and other neurodegenerative diseases. - Analysis of biological and neuroimaging markers in the diagnosis of neurodegenerative diseases and multiple sclerosis. - Experimental therapies applied to movement disorders, dementias and multiple sclerosis.

Neurophysiology and functional explorations of the nervous system

To extend the scientific knowledge of functional aspects of nervous system, integrating both clinical and basic aspects. Our study is focused in both basic and clinical neurophysiology. On theclinical area, our main aims are the characterization of neurophysiological aspects of motor control and pain, the physiopathological aspects of diseases which are co morbid with sleep disorders, epileptic crises and others. On the basic research area, our main aims are the characterization of the physiological cellular mechanisms, both in the ocular physiology and in the mechanisms taking part in the neuronal hyperexcitability in the neuropathic pain. Furthermore, we also work in modulation mechanisms of synaptic transmission.

The research is focused in muscle disease and organic lesions caused by ethanol. The main lines of research are: - Study of the pathogenic mechanisms of alcohol on the organic cells and pathology originated both at neurological and cardiocirculatory levels. - Importance of alcohol in transportation of intracytosolic calcium. - Effects of alcohol on the development of atherosclerosis.- Prognosis of alcoholic myocardiopathy and its best treatment. - Study of muscle and myocardic apoptosis in chronic alcohol consumption. - Study of the physiopathology of muscle diseases, especially muscle immunohistochemical changes, immunogenic changes and angiogenic changes (prognostic factors) of the inflammatory myopathies. - Study of metabolic myopathies, particularly glycogenosis and myoadenylate deaminase deficit. - Study of the myocardic regeneration in the alcoholic myocardiopathy. - Clinical and functional repercussion of chronic fatigue syndrome. - Study of the effects of moderate consumption of wine and the Mediterranean diet in the prevention of cardiovascular diseases.

Cellular biology of pathological processes

The main objectives of team research are: - Study of cellular and molecular basis of mechanisms involved in the regulation of neuronal death, as well as research for neuroprotective and/or substitution intracellular pathways of the neuronal populations that degenerate in neurodegenerative diseases. - To elucidate the molecular mechanisms involved in the interaction of cytoskeleton with the system of cellular endomembranes (particularly Golgi's apparatus), as a structural and functional support of membrane traffic and cell mobility. We also want to clarify how this interaction is altered by cytotoxic treatments such as alcohol. During 2005, we have initiated a new line of research on the structural and functional dynamics of Hirano's neurodegenerative bodies.

Neuropsychology

To study the consequences of brain dysfunctions and injury on cognition and behaviour. The preferred technique used to carry out research is magnetic resonance, in both structural and functional areas. Our group makes use of a neuroimaging analysis laboratory equipped with diverse high capabilities workstations, which use the ANALYZE and SPM medical imaging software. The main lines of research are: - Psychobiological basis of memory impairment related to degenerative and psychiatric diseases. - Cognitive functions of frontal lobe-basal system. - Brain plasticity: long-term consequences of brain lesion in childhood. - Neuropsychology of psychiatric disorders.

Neurobiology unit

The neurobiology unit conducts research on excytotoxic and apoptotic neuronal death molecular mechanisms with the aim to identify new therapeutic targets for the treatment of neurodegenerative diseases. Molecular genetics

The research group is constituted by the Molecular Genetics Laboratory and the Human Genetic Laboratory. Our research activity is framed within the paradigm of current physiology: study of mechanisms of gene expression as a new approach to the study of the organism's functions. Currently, our work is directed to the proteomic and molecular characterisation of spermatozoids in infertile and control patients, on the one hand, and to the study of the mechanisms involved in tumoural angiogenesis and in the transition epithelium- mesenchyme of tumoural cells.

Genetics

The objective of the group is to study and characterise the hereditary disease in its clinical, cytogenetic and molecular aspects. The main lines of research: - Identification of the genetic bases of mental deficiency linked to X-chromosome (XLMR) both syndromic and non-syndromic through CGH-array. - Study of FXTAS (Fragile X Tremor Ataxia Syndrome) a new type of cerebellar ataxia associated to some carriers of the permutation of the Fragile X syndrome (FXS). - Detection of criptical chromosomal unbalances and identification of new phenotypes through techniques of molecular cytogenetics. One is conducted in the framework of pre-natal diagnosis, and the other of post-natal diagnosis. - Study of susceptibility and response to treatment through SNPs in melanoma.

Hemopoietic progenitor transplantation

We want to improve clinical results obtained with the conventional haemopoietic progenitor cell transplantation, to research on the prognostic, clinical and biological factors and to assess new forms of cell therapy. Of particular interest are the activities of the group directed to assess the effectiveness of new prophylactic and therapeutic measures aiming to the reduction of the incidence, morbidity and mortality of the main complications of transplantation, such as graft failure, graft disease against host and infections. Among other aspects of our research we emphasise: the incorporation of new conditioning regimes (aka of reduced intensity); prevention and treatment of the graft-versus-host disease through new drugs; use of monoclonal antibodies; incorporation of the value of reconstitution of dendritic cells; preventive treatment of the cytomegalic disease through specific lymphocytes against the virus and the utilisation of new and more effective antifungals; and the prevention of immediate post-transplantation toxicity. Given the importance of this last aspect, we have started two projects analysingthe value of the endothelial lesion as a prediction of the liver veno-occlusive disease, and a study on urine proteomics, in collaboration with the Nephrology Unit, to make an early detection of severe kidney toxicity. Another section of priority attention is the assessment of new therapeutic indications and new modalities of transplantation. Among the latter is cell therapy through the creation of haemopoietic chimerism as a form of adoptive antitumour immunity for high-risk patients for a conventional transplantation, and autologousand allogenic home transplantation.

Ischemia reperfusion in the transplantation of abdominal organs

Group of lines of investigation based on two experimental models which have demonstrated their utility in clinical application such as stopped heart donor using extracorporeal circulation and the porcine liver transplant model after cuasi-total hepatectomy. Both models allow the study of ischaemia-reperfusion lesions and the factors which may avoid the types of lesions inherent to transplantation.The SFS model is also useful for the study of hepatic regeneration mechanisms.

Surgical treatment of primitive liver cancer and liver metastasis

Group of lines of investigation which have allowed the definition of the optimum criteria for indicating radical treatment of hepatic tumours, including hepatic resection and cadaveric or living donor liver transplantation. From these treatments two sublines of research have been derived aimed at the study of the natural history of the patients undergoing surgery for hepatic metastasis of colorectal origin. New markers which may predict the extension of the disease and the risk of recurrence are under study to thereby guarantee the best selection of patients for surgical treatment: NGAL (Neutrophil gelatinase-associated lipocalin) and proteomic markers.

Endovascular repairment of aortic pathology

Group of national and international projects addressed to the clinical evaluation and development of different endovascular devices applicable in the pathology of the thoracic and abdominal aorta. These include prospective and retrospective analyses of cost-efficacy, safety and efficacy of new generation endoprostheses used for emergency and elective treatment of aneurysms, dissections, traumatisms, penetrating ulcers, inramural haematomas and other pathologies of the aorta. Likewise, programmes of hybrid and combined surgery are performed (endovascular and revascularization of the aortic branches) in the treatment of complex thoracic, abdominal and thoraco-abdominal aortic disease. Other studies are aimed at the evaluation and development of imaging techinques and monitoring for the diagnosis, planning, performance and follow up of aortic endovascular procedures.

New techniques in revasculatization of ischemic limbs

Group of projects aimed at the evaluation of new therapeutic resources of ischemia of the lower limbs which include prospective clinical analysis studies of new endovasclar devices (non-impregnated stents, impregnated stents, pharmacoactive stents...) and tubular prostheses (new materials, association with drugs, antiproliferatives,...) safety and efficacy of cellular and genetic therapy in non revascularizable cases, evaluation of adjuvant drugs to conventional vascular and endovascular surgery, the evaluation of new haemostasis induction products. Moreover, complementary experimental studies have been incorporated in collaboration with the MIT (Massachusetts Institute of Technology) and Harvard - MIT Biomedical Engineering Center, for the evaluation of the biofunctional impact of stents on the arterial endothelium (in vitro and animal models).

This includes a group of experimental and clinical lines of investigation covering the scientific field of organ, tissue and cell donation and related aspects. Applied, clinical and imaging anatomy

This group is focused on lines of investigation in which projects related to surgically-approached anatomy (locomotor, plastic, and reconstructive surgery and neurosurgery), experimental and quantitative neuroanatomy (peripheral nerve regeneration, neuroimaging) and virtual and simulation surgery are developed.

Fisiopathology of fetal and pediatric diseases

The aim of research is the physiopathology of foetal alterations are analysed, especially malformations and growth retardation, at both a clinical and experimental level (models in rats and rabbits). Neurological and biochemical problems are also included, particularly those linked to congenital metabolic diseases and other problems related to different paediatric subspecialties such as endocrinology, cardiology, etc.

Molecular and tumoral embriology

The investigation of this group covers the following lines: - Analysis of polymorphisms (SNPs) in samples of patients with neoplasms with the aim to determine the genetic profiles which are most adequate for antitumoural treatment. ¿ Analysis of microRNAs (miRNAs) in human embryonic and tumoural samples to detect common markers and mechanisms among the initial organogenesis and tumoural phases.

Pharmacoepidemiology

The objectives of investigation of this group include: - The study of the use of the effects of medications in different populations or consumption studies and the effects of medications in the community. - Studies on the use of medications related to the commercialization, distribution and use of drugs in a society with special emphasis on the resulting medical, social and economic consequences. - Studies on drug surveillance which analyse the impact of undesired effects produced by medications in clinical practice. - Analysis of the transfer of results of investigation in clincial practice such as measurement of the impact of the publication of clinical trials on prescription habits.

Health promotion

The objective of this group is to study health care education programmes and advice aimed at the population to improve the level of autonomy and improve the indicators of health (individual, group and new audiovisual technology interventions). In addition, the group evaluates these interventions and those including the implication of the patient in decision-making related to his/her health.
