In the Reasons box, briefly explain why you are
applying for access to the document. For example, “I
am writing a book” or “I am involved in
research and am preparing my doctoral thesis on the
subject of...”.

In the Document box, identify the document to
which you request access (for example, “Mr. X’s
academic record”) and add any other relevant information,
such as the document’s date of publication. If
the document regards a deceased person, indicate how
many years have gone by since the document’s publication
date and the person’s death and note whether you
have next-of-kin status (should you have such status,
official certification will be required).
