Among the Pharmacy officesʼ users, one out of three -polymedicated and aged over 65- patients shows some kidney failure level according to a study carried out by the teaching staff. In addition, the study shows that in this group of patients, one out of two of the prescribed medicines would need a dose adjustment to guarantee the medicine safety in the patient. This consensus guide supports the idea that patientsʼ safety is one of the main factors in assistance quality.

This new tool, aimed at health professionals, has been carried out thanks to the analysis of the most given medicines in community pharmacies. To create an online website for the guide, there has been a knowledge transfer project with the Faculty of Mathematics and Computer Sciences of the UB. This new platform becomes a practical tool which is useful to value the safe use of medicines, propose dosage adjustments and promote collaborative practises among health professionals.
