Canals. El treball, publicat a la revista cient\u00edfica <em>Nature Neuroscience<\/em>, mostra en c\u00e8l\u00b7lules mare pluripotents derivades de pacients (iPSC) i models animals de la malaltia que gens implicats en el desenvolupament del nucli estriat -l\u2019estructura del sistema nervi\u00f3s afectada pel trastorn- ja estan alterats en el proc\u00e9s en qu\u00e8 s\u2019originen les neurones. Segons els investigadors, aquests resultats podrien canviar la manera d\u2019enfocar el tractament de la malaltia, encara sense cura, ja que obren la porta a noves ter\u00e0pies dirigides a la fase pr\u00e8via a l\u2019aparici\u00f3 dels s\u00edmptomes.<\/p>\n\n\n\n<p>La malaltia de Huntington \u00e9s un trastorn neurodegeneratiu progressiu i irreversible originat per la mutaci\u00f3 en el gen que codifica per a la prote\u00efna huntingtina. \u00c9s una malaltia rara o minorit\u00e0ria de car\u00e0cter hereditari que als pa\u00efsos occidentals t\u00e9 una incid\u00e8ncia de cinc a set afectats per cada 100.000 persones.<\/p>\n\n\n\n<p>\u201cTot i ser una malaltia gen\u00e8tica, fins ara es pensava que el desenvolupament del sistema nervi\u00f3s dels pacients \u00e9s correcte fins que el pacient envelleix i comencen a apar\u00e8ixer s\u00edmptomes com alteracions motores (corea, rigidesa, etc.), d\u00e8ficits cognitius i desordres psiqui\u00e0trics. Els nostres resultats indiquen que hi ha indicis subtils d\u2019alteracions en el desenvolupament que fan que les neurones d\u2019aquests pacients no siguin igual que les neurones normals\u201d, explica Josep M. Canals, que tamb\u00e9 \u00e9s investigador de l\u2019Institut de Neuroci\u00e8ncies de la Universitat de Barcelona i de l\u2019Institut d\u2019Investigacions Biom\u00e8diques August Pi i Sunyer (IDIBAPS).<\/p>\n\n\n\n<p><strong>Estudi amb c\u00e8l\u00b7lules iPSC derivades de pacients<\/strong><\/p>\n\n\n\n<p>La primera fase de l\u2019estudi, liderada pel grup de la UB, va consistir en una an\u00e0lisi d\u2019expressi\u00f3 g\u00e8nica durant el desenvolupament estriatal en models animals que despr\u00e8s es va comparar amb l\u2019expressi\u00f3 g\u00e8nica de neurones derivades de c\u00e8l\u00b7lules humanes iPSC. Les iPSC s\u00f3n c\u00e8l\u00b7lules de la pell reprogramades amb factors de transcripci\u00f3 perqu\u00e8 funcionin com una c\u00e8l\u00b7lula mare pluripotent similar a les c\u00e8l\u00b7lules embrion\u00e0ries, amb capacitat de diferenciar-se en qualsevol tipus de c\u00e8l\u00b7lula especialitzada, en aquest cas neurones. Aquest proc\u00e9s permet estudiar els efectes de la mutaci\u00f3 gen\u00e8tica en les c\u00e8l\u00b7lules afectades per la malaltia.<\/p>\n\n\n\n<p>Quan els investigadors van creuar les dades d\u2019expressi\u00f3 g\u00e8nica, van detectar que m\u00e9s de 600 gens responsables del desenvolupament estaven alterats en les c\u00e8l\u00b7lules dels pacients de Huntington. D\u2019aquest llistat de gens alterats, van seleccionar els m\u00e9s importants i finalment es van centrar en una via en concret, l\u2019expressi\u00f3 de la qual es pot controlar amb un f\u00e0rmac: la mol\u00e8cula ISX-9. Aquest f\u00e0rmac es va administrar a c\u00e8l\u00b7lules de malalts <em>in vitro <\/em>i tamb\u00e9 a ratolins model de la malaltia amb efectes neuroprotectors. \u201cEl f\u00e0rmac va revertir alguns dels marcadors: es va constatar un augment de les sinapsis i tamb\u00e9 millores a nivell cognitiu en els ratolins model de la malaltia, els quals milloraven en les proves de mem\u00f2ria\u201d, explica M\u00f3nica Pardo, primera autora del grup de la UB.<\/p>\n\n\n\n<p>\u201cEls resultats demostren que les neurones dels malalts de Huntington, tot i que aparentment fan la mateixa funci\u00f3 que les neurones normals, no s\u00f3n cent per cent iguals i que actuant sobre els gens del desenvolupament alterats es podrien revertir i alleugerir els s\u00edmptomes\u201d, assegura Josep M. Canals. &nbsp;<\/p>\n\n\n\n<p><strong>Nova visi\u00f3 de la malaltia<\/strong><\/p>\n\n\n\n<p>Segons els investigadors, aquest estudi suposa un canvi d\u2019enfocament en la visi\u00f3 sobre l\u2019evoluci\u00f3 de la malaltia de Huntington. \u201cEn comptes d\u2019esperar a l\u2019aparici\u00f3 dels s\u00edmptomes com es fa actualment potser s\u2019hauria d\u2019atacar aquest mal funcionament de les neurones molt abans amb f\u00e0rmacs o induint la funcionalitat de les neurones afectades com ja s\u2019est\u00e0 provant en altres malalties com el Parkinson\u201d, explica Josep M. Canals.<\/p>\n\n\n\n<p>Entre els objectius actuals del HD iPSC Consortium est\u00e0 ampliar l\u2019estudi amb una mostra m\u00e9s amplia de pacients per esbrinar quins s\u00f3n els gens que estan alterats de manera sistem\u00e0tica i homog\u00e8nia en la poblaci\u00f3 de malalts. D\u2019aquesta manera, es podran establir dianes terap\u00e8utiques m\u00e9s precises per desenvolupar f\u00e0rmacs que en un futur puguin aturar la malaltia.<\/p>\n\n\n\n<p>L\u2019estudi s\u2019emmarca en el projecte ADVANCE(CAT), que est\u00e0 integrat a la comunitat NEXTHEALTH coordinada per Biocat i liderat per la UB i Ferrer.
